Care Coordinator

If so, we have an exciting opportunity for you to join our team as a Care Coordinator in our Knottingley branch to join our existing team to make sure that care staff are allocated to visits accurately and the service people receive meets their needs.

As a Care Coordinator, you will play a crucial role in ensuring the smooth and effective delivery of care services to our clients
Your responsibilities will include:

Client Assessment: Alongside the quality assurance team you will be taking part in assessments of clients' care needs and creating care plans.

Scheduling: Coordinating and scheduling care visits, ensuring appropriate allocation of care staff based on clients' needs and preferences.

Communication: Maintaining regular communication with clients, their families, and care staff to monitor progress and address any concerns.

Staff Management: Supervising and supporting a team of care assistants, providing guidance, training, and feedback.

Quality Assurance: Monitoring the quality of care provided and implementing improvements as needed to uphold our standards.

Documentation: Keeping accurate and up-to-date records of client care plans, visits, and any changes in their conditions.

Collaboration: Collaborating with healthcare professionals, social workers, and other relevant parties to ensure holistic care.

On Call:alongside the already established team, you will take part in the on call rota - this will include working evenings and weekends.
Qualifications and Requirements:
- Previous experience in a care coordination role or a similar position within the domiciliary care sector is preferred.
- Strong organisational skills and the ability to multitask effectively.
- Excellent communication skills, both written and verbal.
- Compassionate and empathetic nature, with a genuine interest in the wellbeing of others.
- Proficiency in using computer software for scheduling and documentation purposes.
- Knowledge of relevant regulations and guidelines in the domiciliary care sector.
- NVQ Level 3 in Health and Social Care or equivalent qualification is desirable.
