A new IOH Service is being launched in Staffordshire and Stoke-on-Trent. We are looking for motivated registered practitioners to join our developing service, which will be a partnership provided by Midlands Partnership NHS Foundation Trust (MPFT) and North Staffordshire Combined Healthcare NHS Trust (NSCHT) that brings together the following existing services into a single integrated offer:

- Liaison & Diversion - assesses vulnerable individuals with complex needs being brought into the criminal justice system, having been accused of criminal activity; including but not limited to support with mental health or a learning disability, alcohol or substance misuse, homelessness, financial needs or social difficulties.
- Mental Health Treatment Requirements - Supports the mental health and wellbeing through access to primary level psychological treatments as a sentencing option to address potential contributory factors to someone’s offending
- RECONNECT - seeks to improve the continuity of care of vulnerable people leaving prison by working with them before they leave and supporting their transition to community-based services.

Those who come into contact with the criminal justice system are far more likely to have unmet health and social care needs than the general population. By understanding and addressing those needs, we can reduce reoffending and help our service users on their recovery journey. The IOH Navigator role is a new innovative post, embedded within the custodial, prison and community pathways of the service. Offering screening assessments for a range of vulnerabilities that individuals who come into the contact with the service may experience and for those with greatest need providing an enhanced engagement offer, to support those individuals to engage with the right services to meet their needs. Training on trauma informed care and vulnerability sensitivity will be provided. Work is undertaken under the supervision of a registered senior staff member.

The post holder will be responsible for working within a multi-disciplinary team, with the flexibility of working across Staffordshire, dependent on skill and service need. The Post holder with be part of a multi-disciplinary team, primarily completing screenings of individuals needs at different stages of their journey through the Criminal Justice System.

The post holder will use screening tools to identify health and social care needs. The post holder will assist individuals to navigate health and social care services to address identified needs, will be required to complete onward referrals where appropriate and provide short term intensive support to patients and their families.

The post holder will also be responsible for completing Court Reports outlining individual’s needs that may need be taken into account when decisions about charging and sentencing are made.
