**Job Description - Health & Justice Navigator**
**30 Hours per Week Monday to Friday**
The Health & Justice Navigator will provide face to face, online and telephone support to service users accessing the Reconnect, and the Health and Justice services across Staffordshire and Black Country. Supporting individuals to access welfare benefits, housing and debt advice.

**2. Duties and Responsibilities**
- To be responsible for undertaking an assessment of the patient’s needs in conjunction with clinical staff or other appropriate agencies. This assessment should be based on sound recovery-based principles and encompass the need for social inclusion.
- To monitor the level of engagement by clients with the Health and Justice Service.
- To work within a multi-disciplinary team.
- To identify, monitor and manage risks appropriately within the guidelines of each Midlands Partnership NHS Foundation Trust Sites.
- To liaise with other professionals and agencies to ensure that information regarding the patient’s progress is shared according to the Information Sharing Protocol/s of Midlands Partnership NHS Foundation Trust.
- To perform a range of administrative tasks, such as maintaining client records, record, answering correspondence and dealing with telephone enquiries.
- To ensure confidentiality at all times, adhere consistently to policies and relevant legislation; gaining any necessary authorisation for releasing information prior to doing so.
- To take an active part in a variety of learning opportunities and keeping a record of them (including mandatory training).
- To engage appropriately in supervision.
- To monitor and maintain health, safety and security of others commensurate with your role as defined in the Lone Working Policy.
- To undertake work activities and related training as defined in Health & Safety Policies and Procedures.
- To identify and manage potential risk and document as necessary.
- To assist in maintaining a safe working environment for self, colleagues, clients and others.
- To ensure that colleagues know your whereabouts during working hours.
- To summon immediate help for any emergency and take the appropriate action to prevent a deterioration of the situation.
- To report any issues in the workplace that may put health, safety and security at risk.
- To instigate changes in own practice as knowledge and skills acquired and to adapt to change in a positive manner.
- To report to Service Leads when direction, policies and strategies are adversely affecting service users.
- To recognise the importance of people’s rights and interpreting them in a way that is consistent with policies and legislation.
- To act in ways that:
- are consistent with people’s expressed beliefs and preferences
- acknowledge the benefits of diversity
- value people as individuals
- that is respectful at all times
- To challenge behaviour which infringes the rights of others.
- To identify and take action to address discrimination and oppression.
- To identify and report any significant changes that might affect the individual’s health and wellbeing.
- To obtain consent from individuals before starting to work with them.
- To respect individuals’ privacy, dignity, wishes and beliefs and minimising any unnecessary discomfort.
- To ensure policies and procedures relating to information are strongly adhered to.
- To work within the procedures and protocols of the Midlands Partnership NHS Foundation Trust setting.
**3. Selection Criteria**
Experience of working with Socially Excluded Groups
Experience of working with clients experiencing mental ill health
Ability to work effectively as part of a team as well as being able to work alone and to represent the service externally as required.
Ability to show initiative, good time management skills and to be able to plan and prioritise their own workload, including the execution of any administrative duties.
A good understanding of the recovery model/approach and the principles of social inclusion with a clear ability to be able to translate these principles into practice.
To demonstrate a commitment to the League of Friends’ ethos, policies and procedures, with the ability to understand these policies in relation to the responsibilities of the post.
To demonstrate a good understanding of the needs of people from different racial and cultural groups and the ability to provide services professionally that are sensitive and responsive to the diverse range of people it serves.
Knowledge of service users with drug and alcohol dependency and dual diagnosis.
Knowledge of working service users with pro-criminal attitudes and offending behaviour.
A sound knowledge of the current welfare benefit system and the ability to complete supporting evidence for any claims or appeals on behalf of the patient.
Knowledge of housing allocation.
Have the ability to write letters and clear and concise reports.
