The Locality Coordinator will support the Health Squad Programme Lead with the overall development, implementation and delivery of the service. Due to the nature of the clients, often being part of hard-to-reach groups / communities there will be a requirement to **work flexibly including some weekends and evenings.**

The post holder must have excellent communication skills in order to engage and deliver support for vulnerable populations who are not utilizing health care services. The post holder must have a willingness to undertake mandatory and role specific training within a specified timescale.

1. To line manage staff and support in the development of the staff model for the service based on identified competencies required to deliver the patient journey

2. To support staff in the development and management of their caseloads, providing support and quality monitoring to all aspects of their roles

3. To support staff to develop new services / activities based on identified health need

4. To demonstrate innovative and creative practice to engage with vulnerable clients and utilise support from the wider wellbeing services and relevant clinical and community teams

5. Develop and manage a caseload of clients from the targeted vulnerable population groups to optimise their health and wellbeing

6. Use a personalised approach to care to support clients to seek and undertake appropriate support

7. Work with individuals providing support (digital, face to face, group or home based) through behaviour and lifestyle change, by developing step change personalised action plans and referring and supporting to relevant specialist services

8. Engage with clients and offering them support and brief intervention advice and offering physical health checks, including, finger prick sampling, BP readings, body measurements, and undertaking mental wellbeing assessment tools

9. Source a holistic package of support to assist the development of skills and knowledge, and in particular assisting individuals to progress and achieve their health goals

10. Plan, deliver and evaluate a range of interactive health and wellbeing activities

11. Be responsible for data management of clients, within the locality, including inputting information onto databases and creating reports

12. To signpost or refer individual clients to a range of health improvement lifestyle interventions, primary and secondary care services and/or voluntary and community support.

14. Work with partners and existing groups to develop the referral pathways for support

15. Work in partnership with clinical specialists to offer a multimodal and integrated programme, that avoids clients being passed between services

17. Carry out activity evaluation in line with the project plan and learning from best practice

18. Collate and present verbal and written reports as required

19. Contribute to knowledge sharing and learning before, during and after projects, and to the development of a culture that helps colleagues become confident in using the appropriate technology and develop skills

20. The post holder will be expected to work and communicate with a range of organisations and individuals to achieve the project aims: these will include:

- County Durham Wellbeing for Life
- County Durham and Darlington NHS Foundation Trust
- Other relevant teams within CDDFT e.g. Dietetics, Speech and Language, Physiotherapy, Occupational Therapy, Podiatry Social Prescribing Link Workers, Joining the Dots etc.
- Durham County Council / Area Action Partnerships
- Community and Third Sector Organisations
- Primary Care services, specifically Teams Around the Patient
- Other Wellbeing programmes in County Durham
- Patients, volunteers
- Media

**General**

3. To assist PCP’s marketing & engagement work and use creative techniques to gather views from the communities we support.

4. To be an ambassador for PCP and represent the organisation at a number of different meetings, forums and events.

5. To adhere with financial processes and procedures and ensure that all resources purchased are within the allocated cost code budget(s).

6. To ensure Health and Safety and Safeguarding procedures are followed at all times.

7. To carry out all responsibilities in line with the organisation’s Equality, Diversity and Inclusion Policy.

8. To undertake any training and development deemed appropriate.

To undertake other such duties as are deemed appropriate by Line Manager and Operations Manager.
