As part of this role, you will have the opportunity to provide outreach support and advice to young people, helping them secure appropriate accommodation and maintain their tenancy.
- You will be responsible for managing a caseload of young people, and completing high-quality and comprehensive needs assessments and risk assessments to ensure that they receive the support they need to live independently and achieve their goals.
- You will also have the opportunity to develop comprehensive support and risk plans, identifying individual needs and creating a plan to help each service user live independently within the community and achieve sustained outcomes.
