The Referral Co-ordinator will be responsible for the triage and coordination of throughput through the Glenbourne Unit. This will include; triaging and redirecting calls, gathering thorough referral information so as to aid the nurse in charge in safe clinical decision making and monitoring barriers to discharge across the unit and taking steps to reduce delays. The post holder/s will be responsible for the immediate line management and supervision of the place of safety team and will support the modern matron in the development and operational effectiveness of the department. They will support the nurses in charge of each unit to assist in tasks which impact on the effective running of the unit and might prevent them from undertaking their clinical duties.
These tasks will include supporting in ensuring staffing numbers are adequate, appointments are scheduled and managing phone calls. The referral coordinator will undertake the immediate supervision and management of the place and safety team and will actively participate in the entire place of safety process. This includes managing referrals, assisting in initial risk discussions and support of the workers and detainees as required. This will require excellent communication skills and the ability to form good working relationships between agencies.
The referral coordinator will play a key part in the monitoring of bed availability and development of systems or service changes relating to bed management and occupancy. This will involve being mindful of the progress of people in the unit and proactively consider practical barriers to their discharge including social or safeguarding factors. This may include the attendance of CPA reviews, risk management or internal MDT reviews, and will involve regular communication with other agencies. Managing Incoming communication to the unit: Coordinate referrals to the unit and supporting registered nursing decisions regarding bed management and patient mix.
This referral process will involve taking detailed information and recording effectively through system one. Adhering safely to POS admission criteria and managing process to ensure best experience for person using POS, as well as the safety of POS coordinator and promoting relationships with police and other agencies. The coordinator will managing incoming calls to the unit and will support the clinical team by triaging and allocating tasks, whilst helpfully managing those that fit within the sphere of their responsibility. The coordinator will be expected to demonstrate excellent and supportive customer care during any call, particularly when assisting with the needs of carers who contact the unit.
The coordinator will lead on the location of out of area beds where required for instance due to need for PICU and repatriation. Ensuring Bed availability / throughput: The coordinator will be responsible for being aware of the bed availability across the unit, the expected discharge dates of all inpatients and will monitor progress in conjunction with the MDT process. They will be aware of all out of area placements using the virtual ward on system one and will ensure this is kept up to date, with a regular review of progress of the person and their plan for repatriation. The coordinator will be aware of patients imminent discharge plans and will assist in supporting the clinical team to expedite these plans where they are able.
The coordinator will be responsible for ensuring that 7 day follow ups are monitored and achieved, particularly where a person is not discharged with the support of HTT. The worker will also review discharge plans to ensure completed and documents sent to relevant parties. In line with ensuring throughput within the unit, the coordinator will be expected to attend meetings relevant to their role, for instance attending housing meetings, risk management meetings and CPA reviews as required. The worker will be proactive in identifying barriers to discharge and liaising with relevant parties in order to prevent delays in discharge.
They will work closely with in-house STR workers and external agencies. The worker will ensure that quality and performance monitoring data regarding length of stay and discharge is collated and available monthly. Place of safety related responsibilities: The place of safety worker will have immediate line management / supervisory responsibility for the place of safety team and work closely with the modern matron to ensure staff training, rosters, and absence monitoring and line management supervision is completed in line with policy. The worker will be expected to support workers on a day to day basis and will escalate HR related issues to the matron.
The coordinator will support the referral process as referred to above and will also be the point of contact during the detainees stay for the place of safety worker. The coordinator will link with the nurse in charge of the unit where it is required to do so.
