We currently have a permanent, full time (37 hr) Intermediate Care Coordinator vacancy in Intermediate Care Team (ICT) based in County Hall. The role of ICT is to support safe and efficient discharges from hospital ensuring care and support is in place for those who need it. This role is to support the team of assessors with the administration and management of referrals, adding these to the Eclipse data system, liaising with others regarding the referral information, triaging and prioritising. The role will involve speaking to customers and providing appropriate information, advice and assistance and signposting to appropriate avenues of support, both statutory and 3rd sector.
The role is both administrative with some customer contact and will see you working alongside multiple professionals and organisations to support safe discharge, admission prevention and to support the smooth running of the team.

The Council’s Corporate Safeguarding Policy provides a framework for every member of staff and Service within the Council, setting out responsibilities in relation to safeguarding and protecting children and adults at risk as well as the methods by which the Council will be assured that it is fulfilling its duties.
