We currently have a permanent full time (37 hr) Care Coordinator vacancy in Intermediate Care Team, an integrated health and social care team based in County Hall. The role of ICT is to support safe discharges from hospital ensuring care and support is in place for those who need it. We look to support discharge through various Pathways including “discharge to recover and assess” and supporting into step up/stepdown beds for further assessment and care planning and to prevent hospital admissions where possible. This role is to support hospital related work whether it is discharge related or prevention of admission. It may also be about signposting to third sector or other professionals and organisations where statutory services may not be needed.
