The band 4 will undertake a range of clinically specific patient observations. This will include as an example: Following training, the Band 4 should be prepared and able to safely undertake the following procedures (examples) BLS Nursing assessments and discharge planning Nursing observations Liaison with health and social care providers to plan and deliver discharge plans Social and health planning APs are qualified in their own right and are accountable to the qualified nurse, who asks them to assess any patients or fact find and then documenting in the patients discharge notes any input provided i.e. date, time, name role. Accountable for all practice to the nurses.

Work alongside the nurses, and any/all other professionals, patient and family in relation to complex discharge planning arrangements supporting them where/when needed. Reviews for care home patients, checking the patients have the appropriate equipment in place on discharge and inform the relevant teams if equipment needed. Liaise with Digital health Team, regarding care home patients if they had input with them pre admission. Ensure regular communication with Community teams/social work staff to discuss all appropriate patients on the caseload and expedite discharge.

Ensure that up to date, relevant and accurate information is proactively and accurately maintained on the database for reporting purposes. To act as first line support & link for the patient to the Digital Health service by generating a strong rapport with the patient. To assist in the identification of health needs and participate in the planning, implementation and evaluation of the individuals care under the supervision of the registered Nurses and Paramedics To provide information to patients & carers in relation to any needs and the wider public health remit. To maintain accurate, clear concise & contemporaneous records as per trust policy and communicate to team as appropriate.

Working with patients in their own homes, under taking some investigations when required or installation of equipment To work independently with patients for large amounts of time. Identify and use opportunities to sign post to health and wellbeing services Deliver programs of clinical care & therapy for treatment as part of the plan of care determined by others To deliver clinical & technical interventions, Therapeutic, Rehabilitative, and social care to patients based on their needs To undertake initial assessments Of patients, and provide triage using evidence based care pathways The Band 4 will be able to take a case load of patients planning discharge and assessments for complex case planning, escalating concerns appropriately with supervision. In addition to these clinical responsibilities, the Band 4 will also be expected to adhere to and enact all Trust and local departmental policies in relation to (e.g.) Patient property Safeguarding Deprivation of Liberty Medications Policy Contribution to a safe and clean environment Contribute to concise and legible record keeping Location and Case Mix: The Band 4 will work within the Digital Health Service within the community and hospital site, the case mix will comprise mainly adult patients with a wide range of simple and complex needs. Clinical Governance: The Band 4 will ensure that he/she follows all clinical policies, procedures and guidelines and enact and adhere to all such policies The Band 4 will always and only deliver evidence-based care to all patients.

The Band 4 must recognise her/his limitations in terms of training and development progress and not undertake any duty or procedure beyond the level of her/his competencies even in the unlikely event they are instructed to do so The Band 4 should participate in the evaluation/ audit of clinical standards when requested to do so. She/he must report clinical incidents in line with the policy and comply with all relevant regulations and policies. Informatics: The Band 4 must assure that she/he is fully familiar with the most updated version of electronic record keeping and diagnostic and status displays in the Trust (at the time of writing, the Lorenzo system & EMIS) and maintain accurate contemporaneous electronic records by utilizing this computer software. Confidentiality: The Band 4 may gain knowledge of confidential matters, which may include personal and medial information about patients and staff.

Such information must be considered strictly confidential and must not be discussed or disclosed. Failure to observe this confidentiality could lead to disciplinary action being taken.
