Virtual Ward Nursing Associate

Our General Acute Medicine and Frailty Virtual Ward is a new and innovative Service, that has an agreed admission criteria and works in partnership with supporting care services to deliver acute care in the patient’s own home. The aim of a virtual ward is to reduce length of acute hospital stay and/or avoid hospital admission; this leads to quicker recovery and reduces disruption to patients’, and their carers’ lives. This means we can proactively manage people living with frailty at home, and for those that have a sub-acute episode we can improve a patient’s experience and outcomes. In addition, this means we have more space in our hospitals for acutely unwell patients and reduces the pressure on urgent care.
As part of the Virtual Ward Team, the post holder will be responsible for the implementation of care packages under the supervision of a registered clinical practitioner Carrying out nursing care programmes of work that involve clinical observation, participating in social inclusion activities that supports the patients health and wellbeing and engagement with community services Support in the digital assessment of patients and provide remote monitoring and contact with patients that are suitable for a digital solution. This post is part of the Acute Care at Home service which is responsible for the Unscheduled Care Co ordination Centre (UCCC) Community Rapid Intervention Service (CRIS) and the Community Virtual Ward Service (CVWS)

Key Areas/Tasks
Working under the supervision of a Registered Nurse the key tasks that the post is responsible for is
- Providing holistic and person centred care and support for all people that have been referred onto the Virtual Ward.
- Acting in the best interest of the patients and their family
- Using your knowledge and experiences to make evidence based decisions and solve problems
- Working independently under the management and leadership of the Virtual Ward Senior Nurse Co-ordinator/Practitioner
- Communicating using a range of methods to ensure a caring, sensitive and empathetic manner to patients and their relatives.
- Undertaking a full range of routine care and treatment tasks as directed by the Registered Nurse within the Virtual Ward setting with patients that have been referred.
- Undertaking digital assessments within the patient’s home to see the patient and environment capabilities to provide virtual care remotely
- Act professionally at all times in line with the organisations values and behaviours
- Communicating sensitive information about patients to other members of the health care team and with the consent of the patient to their relatives. This may involve providing information on test results.
- Monitoring the condition and health needs of the patient within the care on a continual basis in partnership with the patient, families, carers and other health professionals
- Perform and record clinical observations including blood pressure, temperature, respirations and pulse, acting within the Trust standards to respond to abnormal results and escalating where necessary
- Escalating concerns on a patient’s condition to senior members of the nursing team, medical staff and initiating referrals to other members of the healthcare team.
- Reporting ward and departmental affairs to the VW Advanced Clinical Practitioner on duty or the VW Modern Matron and to participate in discussions and ward meetings.Ensuring that patients concerns are addressed through liaison with senior staff.
