An exciting opportunity has arisen within Lancashire County Council's Older People Care Services for the position of Referral Officer.

The position available will cover various services across Lancashire, working from County Hall in Preston. Although County Hall will predominately be the base there could be opportunity to work from home or other locations.

The post holder will provide support to conduct preadmission assessments for people leaving hospital who wish to undertake a period of rehabilitation within one of the Older People Intermediate Care Services.

You will be required to manage and co-ordinate residential rehabilitation referrals and maintaining all aspects of the service's data collection/recording requirements. This will include liaising directly with individual residential establishments and other health colleagues and partners to streamline the referral process.

These are full-time, permanent positions and you will be expected to work as part of a 3-week rota on the following days:
W eek 1 - Mon, Tues, Weds, Thurs, Sat - 0900-1700

Week 2- Sun, Tues, Weds, Thurs, Fri, - 0900-1700

Week 3 - Mon, Tues, weds, Thurs, Fri - 0900-1700

Week 4 - Mon, Tues, Weds, Thurs, Fri - 0900-1700

You will be expected to work 1 in 3 weekends. Bank Holidays to be worked as part of your shift pattern.

It runs 16 Care Homes and 13 Day Centres where the team provides support for older people, adults living with dementia and their families.
