Carers Assessment

At n-compass, we are proud to provide Carers with information and support to reduce the impact of their caring role.
We offer true Hybrid Working arrangements, enabling flexibility to work between the community, home and our office in Preston. This is a satisfying and rewarding role with a great team and whilst full training will be provided, we are looking for applicants with the drive and ability to:
- Contact newly referred Carers to induct them to our service and establish their needs.
- Undertake Carers’ Assessments, taking a whole family approach and a review within 6 to 8 weeks.
- Offer support and information appropriate for the carers specific needs, including but not limited to mental health conditions, learning disabilities and dementia.
- Make outgoing referrals to facilitate Carers’ access to other local services/organisations where appropriate.
- Maintain accurate case recording using both Local Authority Case Management Systems and internal recording systems in line with business processes.
- Provide accurate and timely reporting.
- Achieve individual performance management indicators.
- Promote and safeguard the welfare of children and adults.
- Contribute to the newsletter for local Carers and professionals.
- Support the induction, training and professional development of social work students.
