We are currently looking to recruit a Full Time Care Navigation Officer Monday - Friday inclusive of Bank Holiday/Weekend shifts as the service operates 7 days per week. 12 month fixed term from start date

Working across a range of geographical areas you will use your professional skills, often in challenging circumstances to source care placements for Lancashire residents.

You do not need to have any care finding experience to join our team as we will provide you with an induction to develop these skills.

Key Responsibilities

To source care effectively for Lancashire residents

To undertake and effectively manage caseloads showing commitment and enthusiasm for the role

Ability to communicate with internal and external customers effectively

To maintain accurate and up to date records for management purposes and other professionals

To work closely with managers and other professionals to deliver an effective and high quality service

To follow process and procedures as directed

The role requires office working at least 2 days per week.
