**Job Summary**

We are seeking a highly organized and detail-oriented Care Navigation Officer to join our team at Lancashire County Council. As a Care Navigation Officer, you will be responsible for sourcing care packages with external care providers for Lancashire residents using relevant IT systems and software.

**Key Responsibilities**

  • Sourcing care packages with external care providers for Lancashire residents using relevant IT systems and software.
  • Completing all associated administration tasks.
  • Managing an allocation of work.
  • Providing excellent communication skills orally and in writing with internal and external customers and colleagues at all levels.
  • Ability and knowledge of following processes and procedures and working within legal frameworks.
  • Maintaining timely, accurate and up-to-date electronic records and information for management and other professionals while maintaining data security.
  • Working as part of a busy team and in collaboration with colleagues to ensure service targets are met.
  • Developing excellent working relationships both internal and external.

**Working Arrangements**

The service is open 364 days of the year (closed Christmas Day). Successful candidates will participate in a rota determined by the line manager to cover working hours between 8am – 6pm Monday to Friday, as well as weekend and bank holiday working between 9am – 5pm.

We are looking for a highly motivated and organized individual who is able to work independently and as part of a team. The successful candidate will have excellent communication skills, both written and verbal, and be able to work in a fast-paced environment.
