**Purpose of the Job**:
You will be working closely with people who suffering from various mental health conditions to help them recover from their illness and improve their general quality of life. This role includes observing, monitoring and recording client information, providing support.

You will support and empower clients upon discharge from hospital to develop and maintain skills to live independently within the community. To work with mental health practitioners, and other colleagues to support client’s mental health, personal, practical and emotional needs.

**Key Responsibilities**:

- To ensure that the service provides a safe, client focussed environment that utilises a holistic approach to support.
- Provide support to clients to find solutions as issues arise.
- Provide an holistic approach in supporting clients to achieve the goals in their support plan.
- Effectively engage and empower clients to review choices and behaviours and make positive changes to their lifestyle.
- Work in partnership with other services as appropriate.
- Prior to entry into service complete a risk assessment in partnership with the client and referring agent to assess suitability.
- Effective communicator, willing and able to engage with clients
- Have working knowledge of the Mental Health Act and Mental Capacity Act.
- To regularly review support and risk management plans in line with clinical support, ensuring that the support aims are in line with any statutory requirements and the client’s needs and wishes.
- Review the clients suitability for the service at least annually
- Work within the requirements of the hospital discharge plan
- Work in partnership with the housing management team to ensure compliance with the occupancy agreement.
- Ensure that all required documentation is accurately maintained and up to date.
- Ensure that safeguarding concerns are recorded in line with Pivotal’s Safeguarding Adults policy and procedure.
- To record incidents, accidents and near misses in accordance with Pivotal’s Health and Safety Policy.
- Support the Project Manager in ensuring that the service meets it’s key performance indicators, addressing any barriers appropriately.
- Provide data as required by the Support Project Manager.
- Report all maintenance and health and safety concerns to the Housing Management team and support clients to do so.
- Support and encourage clients to attend house meetings.
- Assist your manager in responding to internal / external audit ensuring that any actions are completed in a timely manner.
- To participate in multi agency meetings providing feedback to teams and other managers as appropriate.
- You may be on occasions be asked to work at another site.
- You will need a full driving license, access to a car with business cover on your insurance.
- DBS checks are required for roles where you will be working with or have lone contact with Vulnerable Adults.
