Dementia and Mental Health Support Worker

We are looking for a skilled professional to join our team as a Primary Care Dementia Practitioner in Cornwall.
This dynamic role involves working closely with GP's, complex care teams and dementia specialists to provide specialist support to individuals living with dementia and/or mental ill health.
The successful candidate will be responsible for developing and implementing person-centred care plans, working in partnership with healthcare professionals to ensure holistic physical and mental health needs are considered.
A generous salary of £29,970 - £36,483 per annum is on offer, reflecting the importance of this role in supporting the health and wellbeing of our community.
