Care Coordinator Role

We are seeking a highly skilled and experienced Cornwall Mental Health Practitioner to join our team at the Cornwall Partnership NHS Foundation Trust. As a care coordinator, you will play a vital role in providing assessment, treatment, and care coordination to individuals living with dementia and/or mental health disorders.

The successful candidate will have a strong background in mental health nursing, with experience working with people with dementia and/or mental health disorders. They will also possess excellent communication skills, with the ability to work in partnership with carers/family members and services in the community.

Main Responsibilities:

  • Provide assessment, treatment, and care coordination to individuals living with dementia and/or mental health disorders
  • Manage a caseload under the Care Programme Approach
  • Work closely with carers/family members and services in the community
