Dementia Care Specialist

We are seeking skilled professionals to join our Dementia and Older People's Mental Health Team at the Cornwall Partnership NHS Foundation Trust. This is an excellent opportunity for both Band 5 Primary Care Dementia Practitioners and Band 6 Memory Assessment Nurses to make a real difference in the lives of people living with dementia.
About the RoleIn this role, you will be a key link between primary care and mental health services, supporting patients and their families from diagnosis through ongoing care. You will manage a community caseload, provide education and advice to caregivers, and work closely with primary care professionals to ensure integrated and effective support.
Main Responsibilities- Deliver high-quality memory assessments, contributing to the early diagnosis of dementia and supporting patients and families through the diagnostic journey.
- Manage a community caseload, working collaboratively within a multi-disciplinary team, and providing person-centred care to enhance the lives of those living with dementia.
- A relevant clinical qualification and registration.
The salary for this position is estimated to be around £35,000 - £45,000 per annum, depending on experience.
