About the RoleIn this role, you will be a key link between primary care and mental health services, supporting patients and their families from diagnosis through ongoing care. You will manage a community caseload, provide education and advice to caregivers, and work closely with primary care professionals to ensure integrated and effective support.
Main Responsibilities- Deliver high-quality memory assessments, contributing to the early diagnosis of dementia and supporting patients and families through the diagnostic journey.
- Manage a community caseload, working collaboratively within a multi-disciplinary team, and providing person-centred care to enhance the lives of those living with dementia.
- A relevant clinical qualification and registration.
The salary for this position is estimated to be around £35,000 - £45,000 per annum, depending on experience. As a member of our team, you will also enjoy a range of benefits, including access to training and development opportunities, flexible working arrangements, and a supportive and collaborative work environment.
