We are seeking skilled professionals to join our Dementia and Older People's Mental Health Team at the Cornwall Partnership NHS Foundation Trust. About the Role

In this role, you will be a key link between primary care and mental health services, supporting patients and their families from diagnosis through ongoing care. You will manage a community caseload, provide education and advice to caregivers, and work closely with primary care professionals to ensure integrated and effective support.

Main Responsibilities
  1. Deliver high-quality memory assessments, contributing to the early diagnosis of dementia and supporting patients and families through the diagnostic journey.
  2. Manage a community caseload, working collaboratively within a multi-disciplinary team, and providing person-centred care to enhance the lives of those living with dementia.
Requirements
  • A relevant clinical qualification and registration.
Salary and Benefits

The salary for this position is estimated to be around £35,000 - £45,000 per annum, depending on experience.
