To provide a pro-active service to victims of domestic abuse to keep them safe. Providing a premium service to those assessed at medium risk. The post holder will work pro-actively within a multi-agency setting (including MARAC) to develop comprehensive safety and support plans to help keep victims safe. Key to the success of this role is the use of advocacy and partnership working skills to ensure victims are supported to access services to which they are entitled.

The post holder will need to be highly motivated, flexible and able to work unsupervised on a day-to-day basis.

The post will be covering the North and East Cornwall area, however there may a be requirement to cover for colleagues in other parts of the County during periods of annual leave/ sickness.

**Key responsibilities**:

- Support clients through the referral pathway from crisis to recovery.
- Ensure clients feel supported, listened to and believed.
- Act as a nominated case manager and point of contact for clients and their families.
- Support safety planning, monitor risk (including escalation to high risk IDVA and safeguarding concerns.
- Broker recovery support packages (recovery programs, therapy, peer support, onward referral and access to surgeries across CIOS)
- Provide continuity of support to all clients.
- Maintain accurate and confidential electronic case management records.
- Conduct regular reviews of risk and needs, and use a trauma informed approach to keeping clients and their families safe.
- Review service plans and document progress to assist in a plan to move to independent sustainable living.
- Manage case loads ensuring the service is user centered, cost effective, efficient and responsive to the needs of clients and professionals.
- Facilitate outreach and drop-in services in community-based locations across Cornwall and the IOS.

**Essential requirements**:
