Job Introduction
We are looking for a Band 6 Triage Clinician to join us at our established clinical triage team based from our care coordination centre (CCC) in Peasedown St John.

Our team is expanding due to the development of our virtual ward service which will further underpin our commitment to admission avoidance within BaNES.

Our clinical triage team is responsible for initial triage over the telephone, if successful, you will play a crucial role in managing referrals through highly efficient triage by understand patient healthcare needs. Many of our calls are urgent and require immediate decisive responses. You will work with a range of community services such as district nurses, reablement services and our newly established hospital at home service.

This role would suit somebody with significant knowledge of both urgent care and community-based services. This experience will help manage admission avoidance, hospital discharges, and help ensure patients receive the care they need.

Main Responsibilities
As a Senior Triage Clinician, you will:

- Lead the assessment of a patient’s condition, identifying the most appropriate care pathway.
- Use clinical reasoning skills and knowledge of evidenced-based practice to inform appropriate intervention
- Act as a clinical resource for Band 5 colleagues and Care Coordinators,
- Liaise with professionals, following their advice to ensure clinically safe and effective pathways.
- Maintain accurate systematic documentation of Patient/Health Professional interactions/advice provided.

- Be a registered nurse with NMC registration
- 2 years post graduation experience as a Nurse
- Experience in Urgent Care and Community environments
- Excellent communication skills
- Ability to work as part of a team and liaise with people at all levels

Telephone based triage experience is not mandatory and full training will be given by a well-established supportive team.
