The Post holder will help promote and support the Palliative pathway and patients within the Hillingdon Hospital foundation trust, while taking a leadership role within the Discharge by supporting and guiding the Patient flow coordinators and direct line management of the Administrators in the Discharge hub

The Senior patient flow coordinator will also be the link worker for Homeless patient entering the trust. Ensuring that Duty to refers are completed and the housing department are aware of the referrals and following up on the referrals to ensure that the patient is moved out of the trust in a safe timely manner. The vision will be to reduce the length of stay for homeless patient within the acute hospital, while also leading on proactively discharge planning and coordinating patients that are on a palliative pathway.

The Senior patient flow Coordinator is responsible for actively supporting discharge and care at home for patients with any diagnosis in the last weeks of life to enable a timely and safe discharge from hospital to preferred place of care
- home, hospice, or care home. The Senior patient flow coordinator will also be the link worker for Homeless patient entering the trust. Ensuring that Duty to refers are completed and the housing department are aware of the referrals and following up on the referrals to ensure that the patient is moved out of the trust in a safe timely manner.

The post holder will identify patients recently discharged under Fast-Track Continuing Health Care (CHC), which will evidence the length of hospital stay between decision to discharge and actual discharge. Reduce the number of patients who do not need or want to remain in hospital for end-of-life care by facilitating a robust discharge plan and a responsive, well-coordinated, communicated discharge.

Whilst the initial focus of the role will be to promote and support the patients that are under the care of the Palliative care team, there will also be instances when the ward consultant processes a fast-track pathway for a patient which will also need to be collaborated and supported.

The Senior Patient flow coordinator will also be expected to support junior colleagues and provide a level of expertise in the discharge process.

The Hillingdon Hospitals NHS Foundation Trust is the only acute Hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics and maternity services. The Trust’s services at Mount Vernon Hospital include routine day surgery, delivered at a modern treatment centre, a minor injuries unit and outpatient clinics.
