As a Care Coordination Specialist, you will be responsible for providing a central coordination role for patient care planning, working closely with GPs and other primary care professionals within the PCN. You will also support the coordination and delivery of multidisciplinary teams within the PCN and work collaboratively with other care coordinators, social prescribing link workers, and health and wellbeing coaches. If you have excellent communication skills, good IT skills, and empathy, we encourage you to apply for this exciting opportunity. Salary: £30,000 - £40,000 per annum.
