We are seeking an experienced and skilled Nurse to join our team as a Respiratory Care Specialist in the Transformation Unit at The Hillingdon Hospitals NHS Foundation Trust.

About the Job

This is a unique opportunity for a dedicated professional to develop their skills in delivering high-quality care to patients with respiratory conditions. As a key member of our team, you will be responsible for planning, evaluating treatment, and providing clinical assessments to patients in various settings.

You will work closely with multidisciplinary teams, including general practitioners, practice nurses, and other healthcare professionals to ensure seamless care delivery. Your expertise in non-medical prescribing, ARTP certification, and community working will be valuable assets to our team.

Main Responsibilities
  • Treatment Planning: Develop and implement individualized care plans for patients with respiratory conditions, incorporating evidence-based guidelines and protocols.
  • Clinical Assessments: Conduct thorough assessments in patient homes, ambulatory/secondary care clinics, and other settings, making informed management decisions when necessary.
  • Integrated Care: Participate in multidisciplinary meetings and case conferences to coordinate care and ensure optimal outcomes.
  • Admission Avoidance: Support schemes aimed at preventing hospital admissions by assessing acutely unwell patients in various settings, including patient homes, general practices, and community sites.
  • Covid Virtual Ward: Provide virtual assessments and monitoring services to support patients with respiratory conditions during the pandemic.

Requirements
  • Diploma or degree in Nursing
  • Current NMC registration
  • Evidence of Continuing Professional Development (CPD)
  • Full UK driving license and access to a car
  • Knowledge and experience in respiratory care
  • Non-medical prescribing qualification
  • ARTP certification
  • Experience in community working
  • Audit and research experience
