Discharge Coordinator/Staff Nurse

About the Role
We are seeking a highly skilled Neonatal Nurse to join our team as a Discharge Coordinator. This role offers the opportunity to work in a level 2 Neonatal Unit and Transitional Care Unit based in the postnatal ward.
The successful candidate will have experience in neonatal care and a passion for discharge planning. They will work closely with the multidisciplinary team to ensure seamless discharge of babies with complex needs.
Key Responsibilities
- Develop and implement discharge plans for babies with complex needs
- Collaborate with the multidisciplinary team to ensure smooth discharge processes
- Provide specialist advice and guidance to colleagues on discharge procedures
- Supervise and teach junior staff on discharge planning and execution
- Work closely with external agencies to ensure timely and safe discharge
