**PRIME OBJECTIVES OF THE POST**

The post holder will be a member of the CLICS team. The post holder will manage and lead a team of CLICS Community Health Champion volunteers within the given locality. They will support the wider team to deliver group and person-centred care through engaging community members into volunteering. With the wider team members, they will work with individuals and groups utilising untapped community resources, to support the facilitation of self-help groups and other innovative approaches to enable people to better manage their own care.

It is expected that the post holder will also generate and lead on both delivering and sourcing training for volunteers. The post holder will also provide group supervision to the volunteers they engage.

In addition, the post holder will work collaboratively with the wider team to develop the service across Community Partnership, auditing activity, reviewing progress and identifying where solutions are best delivered at local or city-wide level.

The Health Champions are seen as fundamental to the success of CLICS, local people able to support family, friends and neighbours through signposting to voluntary and community groups that might support people’s health and wellbeing. The Health Champions will be a practical resource to help connect people into services at a local level.

**MAIN DUTIES AND RESPONSIBILITIES**

The Post Holder will:
**Leadership**
- Take the lead for volunteering in CLICS geographical locality
- Work with the other CLICS Volunteer Coordinators to develop and source training for the CLICS Health Champions
- Ensure links with Living Well Academy
- Provide supervision and support for CLICS Health Champion volunteers
- Support CLICS Health Champions to develop support groups/ networks
- Ensure that all audit and reports are undertaken as requested by the peer support team leader
- Ensure any databases are accurately updated and maintained.
- Represent the service at meetings both in and out of the locality.

**Mapping & Promotion**
- Have strong links with the voluntary sector, supporting the voluntary and statutory sector to network and improve partnership working
- Support the CD worker to map both statutory and voluntary services in the community that promote health and wellbeing
- Promote CLICS support and RIC services within the locality, both for users and clinicians
- Work with the CLICS Health Champions to develop and deliver awareness raising events that support people to improve their health and wellbeing and improve self
- management
- Disseminate marketing materials to a wide range of audiences
- Promote the service via social media, newsletters, and bulletins and through thewebsite

**Needs assessment**
- Listen to patients/ residents and be aware of areas where there are gaps in service provision
- Work with partners / GP practices/ community members/ volunteers and other organisations in the area to identify gaps in provision

**Increasing resources**
- Work with CLICS Health Champion Volunteers/ patients/ residents and other services to help fill gaps in service provision within current resources
- Work with their CLICS Health Champion Volunteers to develop and support peer support groups

**Team responsibilities**
- In conjunction with the CLICS wider team identify un-met needs that benefit from City-wide responses, looking to cost neutral solutions
- In conjunction with the other Volunteer Coordinators in CP4/6 identify training needs and facilitate delivery for volunteers
- Contribute to audit and reporting mechanisms within the service
- Act as a resource/support for other team members
- Contribute to the dissemination of learning gained, sharing good practice

**All post holders are required to**:

- Attend training, external meetings and team meetings

- Assist people from marginalised groups to access quality care

- Maintain personal and professional development in order to maintain skill levels and participate in the appraisal process and any training and development that is recognised and agreed with the line manager
- Work within the operating hours of their base. Some evening and weekend work may be required
