Care Service Coordinator, Assessment and Admissions

We are currently recruiting a highly motivated Care Service Coordinator to oversee Assessment and Admissions. We provide a high standard of individualised and specialised care to young adults with learning disabilities to help them live independent and fulfilling lives. As part of your work with Abilities Development, you will be building great relationships with the families and helping them with their own planning and goals.
The successful Care Service Coordinator will be a multi-skilled staff member. You will work with Social Workers, Nurses, Therapists, potential and current clients and their parents/carers to gather information to enable the Management team to decide the support required.
As a Care Service Coordinator, you will undertake the full range of tasks associated with assessment, admissions, care planning and reviews. Your aim is to ensure that we are giving people the right support in the right environment to help them remain as independent and socially included as they want to be.
You should also remain aware of all relevant regulation and may assist in meetings with regulatory bodies that monitor Health & Social Care. As part of your Care Service Coordinator duties, you will be required to carry out on-call / out of hours duties on a rotational basis.
Being our Care Service Coordinator is a varied and important role. You will also:
- Liaise with schools, colleges and other professionals regarding the needs and current ways of support for potential clients.
- Liaise with Senior Coordinator and manager to discuss each case for admissions
- Compile assessment packs and all documents related to client admission.
- Show willingness to attend training and support staff in developing their understanding of assessment.
**Ideal Care Service Coordinator skills and experience**
- NVQ level 3 in Care, education or working with children and young people. If not, you should be willing to pursue an NVQ or equivalent qualification.
- Proven experience of working with learning disabilities.
- Previous experience working in an educational/healthcare environment with knowledge of SEN Code of Practice.
- Rounded staff management skills and with a good track record of success within the teams you have managed.
- Proven track record of team training, development and organisation.
- Excellent communication skills and effective organisational capability.
- High degree of flexibility when responding to the changing needs of our Clients
- A strong can do attitude and hands on approach.
