Patient Pathway Coordinator/ Medical Secretary

An exciting opportunity has arisen for a fixed term maternity cover post as a Band 4 Patient Pathway Coordinator working in the Cardiology Arrhythmia Service within the Cardiovascular Directorate.

The Patient Pathway Coordinator will lead on validating pathways and ensuring patients are not lost to follow-up. By working closely with a wide range of clinical and managerial staff and providing high standard of administrative support, the post holder will ensure patients receive treatment as soon as possible and ensure national targets are met.

Monitor and review patient care and/or treatment progress using the Patient Tracking List in line with Trust and local policies.

Identify from the Tracking List the next steps in the patient care pathway in line with clinical need (peer review).

Work as a team leader, providing a comprehensive secretarial/administration service to a number of consultants, clinicians and specialist nurses in respect to their general medical duties and specialist work.

Type all clinical letters, which may include those of a distressing or emotional nature, as well as discharge summaries or other relevant correspondence and documents are required.

The Patient Pathway Coordinator / Medical Secretary will be an exciting and varied role, working closely with the Assistant Service Manager and booking team to support with management of patient pathways, whilst taking on Medical Secretarial duties to support clinical teams with the administration of clinical letters and documentation. The successful applicant will have the opportunity to work closely with administrative and clinical colleagues for a smooth delivery of patient care.

Strong communication skills and the ability to work with others within different roles within the Trust are key to this position, as is strong administrative experience and the drive to improve patient care.
