Patient Pathway Coordinator

We are seeking to appoint a full time Patient Pathway Coordinator within the Urology Department.
The post holder will be responsible for providing administrative support, including clinical letter tying to the clinical team. The role will focus on supporting a positive patient experience within the service with a key emphasis on patient pathway tracking and validation to achieve RTT standards. The post holder will work closely with the service team and receive support and training as required to fulfil the role.
The role of the Patient Pathway Coordinator is to work closely with clinicians and Service Managers to ensure that the care pathway of all patients is effectively managed.
The post holder will be required to act as a flexible administrative resource actively supporting patient pathway improvement projects across the Trust.
The post holder will have direct patient contact working with the wider multidisciplinary team to ensure that patient care is coordinated and delivered with minimum level of difficulty to our patients, in the service delivery of 18 week and cancer waiting time pathways and other performance targets.
This role will also ensure the timely turnaround of patient letters including typing as and when necessary.
Applicants must have a good standard of general education and possess a secretarial qualification or equivalent. Previous experience of working to targets dictated by national NHS Trust agendas, for example, the Referral to Treatment pathway is desirable as well as demonstrable experience of the Trust Patient Administration Systems. Applicants must have the ability to problem solve, cope with a large workload and prioritise according to deadlines.

Urology is a surgical speciality, covering the diagnosis and treatment of disorders of the kidneys, ureters, bladder, prostate and male reproductive organs.
- To be responsible for the management of the patient pathway, from referral to first outpatient appointment, through diagnostics, treatment planning, treatment, and follow-up, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process.
- To have a sound understanding of 18 week and cancer waiting time targets, rules and procedures, and to work proactively to ensure that all targets are maintained.
- To be responsible for the efficient turnaround of clinic and patient letters including the editing and typing as and when required within remit.
- To be responsible for accurate and up to date information in medical records and accurate patient letters within remit.
- The post holder should provide a weekly update to Service Managers so they are aware of any pathway delays or potential breaches of waiting times targets.
- To routinely attend performance review meetings and ensure that agreed actions and outcomes are requested, documented and monitored.
- To be responsible for booking and sending out-patient, admission / pre-assessment appointment letters and telephoning patients as required.
- To schedule outpatient appointments and inpatient operating lists, ensuring that clinics and theatres are utilised appropriately and waiting lists are managed effectively.
- To establish and maintain strong communication links with the clinical team, attending weekly planning meetings and ensuring lists are managed in a clinically appropriate manner as guided by the team.
- To liaise with consultants and their teams to monitor and ensure correct case mix on theatre lists, making appropriate staff aware of any changes, including Site/Bed manager and Medical Records staff using the agreed protocols / shared files.
- To ensure patient cancellations are allocated new TCI dates within the specified time limits.
- To manage pre-assessment clinics, working in close conjunction with pre-assessment staff to ensure the smooth running of clinics.
- Monitor elective surgery waiting times, proactively identifying risks, and taking corrective actions to ensure relevant targets are not breached, in conjunction with the Service teams, and clinical teams.
- Deal with non-clinical queries from patients, their carers, general practitioners and members of the admin & clerical team.
- Escalate potential breaches of waiting times or patient pathway bottlenecks to the Access Manager and Service Management Teams as appropriate.
