Working closely with the Health Co-ordinator team, Social Prescribers and clinicians to support the wellbeing and care needs of our patient population. You will have access to general advice and support and will also have clinical team oversight or supervision from the Practice Manager where necessary. Main Responsibilities To provide wide-ranging, proactive and efficient administrative support to Cheriton Bishop & Teign Valley Practice To be responsible for the administration, co-ordination and monitoring of dedicated or targeted clinical campaigns To work closely with Practice Administrator, nursing and coding teams to assist the practice in meeting contractual targets To utilise population health intelligence to proactively identify and work with a cohort of patients to delivery personalised care, including through recalls for specific patient groups (eg, immunisations, health checks, reviews etc) To discuss patient-related concerns with the relevant staff and be supported to follow appropriate safeguarding procedures To holistically bring together all of a persons identified care and support needs and explore options to meet these within a single personalised care and support plan (PCSP), in line with PCSP best practice, based on what matters to the person To help people manage their needs through answering queries, making and managing appointments and ensuring that people have good quality written or verbal information to help them make choices about their care To provide co-ordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches and other primary care professionals.

To work with GPs and other primary care colleagues to identify and manage caseload of patients, and where required and as appropriate, refer people back to other health professionals The post holder will be aware of the various statutory requirements and will assist in implementation as directed by the practice manager. There is a need to work closely with Health Co-ordinators and clinical staff to ensure the smooth running of the practice, reporting any problems encountered to the relevant person. Training requirements will be monitored by annual appraisal. Personal development will be encouraged and supported by the practice.

It is the individuals responsibility to remain up to date with recent developments. PRINCIPAL DUTIES AND RESPONSIBILITIES Health & safety: The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include: Using personal security systems within the workplace according to practice guidelines Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks Making effective use of training to update knowledge and skills Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and keeping them free from hazards Actively reporting health and safety hazards and infection hazards immediately when recognised Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role Undertaking periodic infection control training (minimum annually) Reporting potential risks identified Demonstrate due regard for safeguarding and promoting the welfare of children. Production of performance and quality information To ensure that documentation (electronic and paper-based) exists to support performance standards across the full range of performance-based activity QOF, Enhanced Services etc.

To assist in the production of information for clinical audit. To undertake ad-hoc work related to the performance of the practice under the guidance of the practice manager. Data quality To work within the clinical computer system to improve data quality, using the expertise of other data staff where appropriate.
