To work closely with MDT Coordinators, Cancer Trackers, Clinical Offices, Appointments Centre, Central Booking Office, Diagnostic and Therapy Departments across all specialties to ensure patients are actively managed through the cancer pathway. To work closely with the Cancer Lead of the Respiratory Department to ensure patients are actively managed through the cancer pathway. To monitor patients pathways from referral, through diagnostics, treatment planning and treatment, highlighting any potential delays in the pathway. To ensure accurate data collection to support the timely management of patients along their pathway in order to identify and respond to avoidable delays.

To ensure that appropriate requests for investigations are monitored following the clinicians instructions. If diagnostic delays occur or appear imminent liaise with the relevant service for action. To liaise with member of individual specialties management teams and the Trust to ensure coordination of all elements of patient pathways. This may include highlighting patient target dates etc to facilitate investigations in accordance with the timed pathway and escalation of deviations and exceptions.

To provide accurate, up to date information to the MDT following confirmation of cancer diagnosis to ensure treatment within national cancer targets. To transfer patients with a non-cancer diagnosis on to the appropriate clinical pathway for follow up and treatment where appropriate. To maintain up to date knowledge of all cancer standards, targets and processes relating to the patient pathway and to be a point of reference for administrative teams across the Trust. To demonstrate excellent communication skills.

To ensure effective liaison between all members of the MDT and with other Trusts, GP practices, patients and carers. To ensure all information relating to the patient pathway is communicated effectively and by the most appropriate medium. To provide local training and support to other staff within the Trust regarding 28 Day Faster Diagnosis Standard and other cancer standards. Information & Reporting To ensure all information relating to the patients pathway is clearly documented in a timely manner on the Somerset Cancer Registry database to aid all users identify next actions for each patient.

To attend and present weekly PTL meetings and provide detailed patient level information. Technology or System Support To have a full understanding of the Information Systems in use for the management of patients pathways. Service improvement To help identify areas where the patient pathway could be improved and to work with the specialties to implement change and improvement. Professional Development/Education To participate in the induction of new staff, and to educate others about the role of the pathway coordinator.

To participate in performance and development review. To undertake any further training relevant to the post or personal development. Communications and Engagement To provide updates on the delivery of the 28 day standard at Surrey and Sussex Alliance pathway meetings. Face to face and telephone communication and engagement with cancer patients and families.
