To be fully conversant with patient pathways across site specific tumour groups to ensure that patients are treated equitably, reaching targets for holistic needs assessments and diagnostic tests. To liaise with MDT coordinators and across the Trust to monitor the coordination of all elements of the patient pathway. To provide support to the team to ensure continuous development, implementation and monitoring of changes to the patient pathway. To be conversant with all reports available on the Somerset database to manage the patient pathway.
Responsible for coordinating rapid re-entry for specialist review against a clinically approved triage model. Development of survivorship and wellbeing services. Support the delivery of the Recovery Package in line with the Cancer Alliance. Support and participate in audit processes and service development.
To identify delays in the patient pathway and to follow the trust escalation policy to raise awareness to the Team Lead for Macmillan and Lead Cancer Clinician as appropriate. Working in a team environment to provide appropriate support/advice to other secretarial/clerical staff as necessary. Build relationships with a wide range of staff both internally and externally to undertake job role. Ability to promote patient empowerment and self-management strategies.
Deliver self-management advice, support and know when to escalate to clinical team.
