**About the Role**:
The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service, working on requirements for the PCN Direct Enhanced Service Contract (DES).

**The main duties of the role include**:

- supporting the surgery in coordinating all key activity, including access to services, advice, and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
- supporting digital initiatives.
- ensuring patients are invited and have access to appointments for vaccinations including flu and covid boosters.
- assist in collecting and submitting of relevant data.
- support surgery and patients during the long term condition review process.

If you have experience of care planning, with strong communication skills and excellent organisational experience, then we would love to hear from you.

The ideal applicant will ideally have the following skills, experience and qualifications:

- Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups;
- Strong listening and communication skills;
- Familiar with local resources and services, including how to access them;
- Motivated by helping people, with care and empathy;
- Able to deliver person-centred support in a non-judgmental way;
- Excellent organisational skills with attention to detail;
- Working within a team environment;
- Respect patient confidentiality at all times.

Bury St Edmunds Primary Care Network consists of five surgeries and serves some 65,000 patients from the town and surrounding villages. Our vision is to deliver the highest standards of care, with equality, dignity and respect, to our patient population. Our aim is to create a system where member practices work closely together in collaboration with health and social care, voluntary sector, community groups and local people, to deliver inclusive and personalised care.
Bury Primary Care Network is a growing PCN with existing roles including, care coordinators, social prescribers, clinical pharmacists, paramedics, mental health practitioners, health and wellbeing coaches and pharmacist technicians that support GPs and other clinical teams to enhance patient services in the local community.

The care co-ordinator role will contribute to improving the quality of patient care across the network of practices. You will be joining an enthusiastic team of clinicians and administrators and will be well supported with on-going professional development (training support, CPD and peer support).
