An exciting opportunity has arisen for a Band 6 Community Psychiatric Nurse (CPN)/ to join the Primary Care Liaison Service Team. You would be supported by the team to complete a set of competencies and with additional Trust-wide support with reflective sessions and working groups to develop your skills.

The Merton Primary Care Liaison Team is a community-based service for working-age adults between 18 to 75 years, who have experienced severe and enduring mental illness. The Merton Primary Care Liaison Team works collaboratively with clients, their carers and networks with the aim of minimising the impact of mental disorder has on their lives. The team will work closely with Primary Care and will promote recovery and support to and from GP surgeries. This will include medication management and practical support for example connecting people into wider community services including vocational needs, employment, health promotion, peer support and other areas which will enhance the persons recovery.

Main duties of the job

The successful candidate will be able to offer intensive engagement work and recovery-orientated interventions. You will work with a complex caseload of clients with varied mental health conditions. The role primarily involves developing treatment and interventions for health conditions within a recovery model of care to support people to move from Primary Care into secondary mental health and to successfully transfer them back to their GP with the right preparation over an appropriate time frame. Successful candidates will need to demonstrate an understanding of client's mental health needs and how this connects with the wider Community Transformation Programme.

You will be able to develop and enhance the therapeutic care of individuals within the community. The role involves among other things, completing comprehensive assessments, risks assessments, drawing up meaningful care plans, risk management plans and delivering appropriate and therapeutic interventions. You should be committed to MDT and partnership working, establishing good networks with internal trust and external colleagues including VCSE agencies.

Job responsibilities

Working autonomously within the parameters of the role leading and supporting junior staff and wider care team to implement the delivery of quality nursing care, contributing to the physical, psychological, social, recreational and spiritual care in a recovery-focused way through therapeutic engagement and activities, and taking steps to address any issues that arise.

Leading and supporting/teaching junior staff on risk assessment and risk management that involves making decisions based on knowledge of the research evidence, knowledge of the individual service user and their social context, knowledge of the service users own experience and clinical judgment.

Advocating for service users needs and rights within Trust policy. Encouraging and empowering service users to have an optimum level of responsibility for their individual programme of care.

Escorting service users, as required, in line with Trust policies.

Demonstrating an enhanced understanding of Mental Health signs and symptoms and an understanding of how these may present in practice including recognition of impact on patient presentation and behaviour including risk factors. Supporting and teaching junior staff to develop understanding of mental health signs and symptoms and develop appropriate responses to these in practice.

Recognising and responding appropriately to challenging behaviour in line with Trust policies and training. Leading and coordinating junior staff and the wider care team in the management of difficult situations and de-escalation and developing resilience and strategies to manage, including on leading staff and patient de-briefing sessions.

  • Registered Adult Nurse and/or Registered Mental Health Nurse and/or Registered Learning Disabilities Nurse

  • Experience of researchbased/reflective practice, e.g. understanding and ability to use supervision to reflect on skills, attitude and knowledge and develop these by using evidence based practice
  • Significant relevant clinical experience
  • Evidence of continuing professional development
  • Experience as a preceptor / mentor
  • Knowledge of clinical governance and audit
  • Knowledge of relevant Professional and Clinical legislation

  • Experience of taking responsibility for a ward / clinical unit.
