Discharge Coordinator/Network Lead

We are seeking two Band 7 Discharge Coordinators - these are new roles within the Hospital Division.
The Discharge Coordinators will add capacity to the acute in-patient wards through a programme of active leadership in relation to daily actions and tasks to promote patient flow.
Each Discharge Coordinator will oversee flow on four wards, and will have an Assistant Practitioner in situ in each ward to support with patient flow tasks and who will report to them directly.
- Co-ordinate efficient and effective patient discharges
- Ensure all discharge plans are established with the wider care network within 72 hours of admission.
- Liaison with the Discharge Facilitation social workers
- Attendance and verbal contribution to the daily access and flow, bed management meetings, and within weekly MADE meetings.
- Overseeing care for patients who may have been placed out of area.
- Engagement with voluntary and community sector organisations.
- Leading on the service being data driven and use of data to compile activity reports and drive productivity by reducing length of stay within the acute wards.
- Supervision, appraisal and performance management of staff which are line managed within the post.
- Cross cover for peer discharge coordinator
- Responsibility for the team budget
- Undertaking of investigatory activity as needed (PSIRF, complaints activity etc)
In order to meet the needs of the new Partnership services you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Enfield and Harringay Trust and Camden and Islington Trust. Ensuring that discharge plans are established with the network of care around service users within 72 hours of admission. Barriers to discharge will be established for early resolution.
Connecting with social care led interventions delivered by the Discharge Facilitation Social Workers.
Key point of contact for the Patient Flow Team to aid their overall responsibility for coordination of access.
Contributing to the daily patient flow meetings and MADE meetings.
Oversee the care of service users in out of area placements.
Strengthening engagement with carers to support their key input into care planning and discharge planning.
Strengthen engagement with the VCS to maximise their input into care planning and sustaining recovery in the community.
Lead on team being data driven, strengths-based and recovery orientated. Least restrictive practice principles will be supported by a positive risk decision making approach.
Ensuring data is collected to support evaluation of impact and producing regular reports.
Supervising and managing a small team of Band 4 staff ensuring that they provide clinically led coordination of meetings that support network engagement within care planning and discharge planning.
Providing the Head of Service with regular assurance on all activities. Person Specification Qualifications Essential
- Degree level education or equivalent experience
- Professional registration or equivalent experience
- Evidence of continuing professional development in relation to mental health practice
- Evidence of Quality Improvement training Evidence of management training/Abilities
- Ability to influence clinical decisions using a strengths-based, positive risk taking approach.
- Excellent interpersonal skills with other professionals, service users and carers.
- ICT literate with the ability to utilise data to inform action (ability to use the EPR, Word, Excel, Outlook & MS Teams)
- Excellent clinical skills in risk assessment, risk management planning and care planning.
- Ability to establish and manage clinical audit and/or quality improvement project activity.
- Substantial previous clinical experience in an NHS inpatient mental health setting.
- Understanding of discharge planning and patient flow across health and social care systems.
- Understanding and insight into national guidance and policy relating to hospital discharge.
- Experience of work in a variety of settings including community services.
