Role Overview:
The Hospital Referral & Discharge Coordinator plays a crucial role in ensuring that patients transition smoothly from hospital to home. This position requires a keen understanding of the social factors that may impede a patient’s discharge when they are medically cleared to leave the hospital.

Key Responsibilities:
As a vital member of the healthcare team, you will:
1. Accept referrals from various healthcare facilities, including hospitals and emergency departments.
2. Assess and assign a dedicated support worker to complex cases to facilitate timely discharges.
3. Assist clients in overcoming barriers to discharge, such as social isolation or inadequate living conditions.
4. Collaborate with multidisciplinary teams, including hospital staff and social workers, to ensure comprehensive support for patients.
5. Provide practical assistance in navigating housing and benefits applications.
6. Advocate for clients, connecting them with resources that promote health and well-being.

The ideal candidate will possess:
- A strong background in case management and experience working with diverse populations.
- Excellent problem-solving skills and a creative approach to overcoming challenges.

- A full driving license and access to a vehicle, as travel may be required.
