About the Role:

As a Discharge Coordinator, you will play a vital role in ensuring smooth transitions for patients leaving our care. You will collaborate with multidisciplinary teams, including doctors, nurses, social workers, and therapists, to develop individualized discharge plans that meet each patient's unique needs.

Key Responsibilities:

  • Coordinate the discharge process for patients, ensuring timely and safe transitions to appropriate care settings.
  • Collaborate with healthcare professionals to assess patient needs and develop comprehensive discharge plans.
  • Communicate effectively with patients, families, and other stakeholders involved in the discharge process.
  • Liaise with external agencies, such as social services and community support organizations, to facilitate seamless transitions.
  • Monitor patient progress post-discharge and provide ongoing support as needed.

Skills and Qualifications:

The ideal candidate will possess strong communication, organizational, and problem-solving skills. A background in healthcare administration or a related field is preferred. A commitment to providing compassionate and patient-centered care is essential.
