Job Summary

We are seeking a highly motivated and experienced Senior Discharge Coordinator to join our team at Whittington Health NHS Trust. The successful candidate will be responsible for coordinating the discharge process for patients, ensuring a smooth transition from hospital care to community care.

The ideal candidate will have a strong understanding of national best practice in discharge systems and processes, as well as knowledge of clinical governance and risk management systems.

Key responsibilities will include:

  • Coordinating discharge planning with hospital, community, social, and health service colleagues
  • Supporting the training and development of ward staff to ensure best practice in planning safe and timely patient discharges
  • Working with the Integrated Discharge Team to provide support and advice to ward teams
