About the Role

We are seeking a highly skilled and experienced Discharge Coordinator to join our dynamic team at Lewisham and Greenwich NHS Trust. As a key member of our Discharge Team, you will play a vital role in ensuring the smooth and efficient discharge of patients from our acute hospital.

Key Responsibilities
  • Manage a personal caseload of wards and coordinate complex discharges from the Acute hospital.
  • Ensure that all relevant paperwork is completed by the appropriate persons to ensure an efficient and timely discharge.
  • Attend daily meetings with the multidisciplinary team to discuss patient care and discharge plans.
  • Provide education and training to members of the multidisciplinary team on discharge planning and coordination.
  • Monitor performance associated with discharge, identifying gaps in service provision that may have a detrimental effect on length of stay, delayed transfers of care or patient experience.
Requirements
  • Significant experience in an NHS acute setting.
  • Professional Clinical qualification or equivalent experience in case management in primary, secondary or social care.
  • Ability to work unsupervised using own initiative using evidence and experience to inform safe decision making.
  • Good written and verbal communication skills: ability to communicate complex and sensitive information with patients and carers, where there may be barriers to understanding.
