Essential Qualifications:

The successful candidate for the Hospital Referral & Discharge Coordinator role will:

  • Possess an understanding of the social dynamics that may impede a person's timely discharge from a healthcare facility once they are medically cleared.
  • Have a comprehensive skill set and knowledge base, including familiarity with housing support, legal aspects of housing, financial aid, welfare benefits, and universal credit.
  • Demonstrate experience working with a diverse clientele.

Role Overview:

The primary objective of this service is to facilitate a safe and prompt return home for individuals who no longer require hospitalization for clinical reasons or those who face environmental or social challenges in returning home independently.

As a Referral and Discharge Coordinator, you will be stationed at Dorset County Hospital in Dorchester and will be tasked with:

  • Receiving referrals from all Dorset Hospitals, Emergency Departments, General Practitioners, and Urgent Community Response Schemes.
  • Assessing and assigning a First Point Discharge and Community Support worker to complex cases.
  • Assisting clients at the initial point of contact to address discharge barriers such as social isolation, property conditions, or environmental concerns within their homes.
  • Collaborating with all Dorset Hospitals, including both community and acute services, and working closely with hospital and social work teams, including ward staff, discharge teams, emergency department teams, cluster teams, patient key workers, and social workers.
  • Providing practical assistance, such as guiding individuals through housing or benefits applications.
  • Encouraging clients to access resources that promote positive health and well-being, including addiction services, peer support, or social groups.
  • Engaging in partnership work with various agencies and organizations, often advocating on behalf of clients.
