About the Role

We are seeking an experienced and skilled Oncology Discharge Coordinator to join our team at University College London Hospitals NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in ensuring the smooth transition of patients from our inpatient oncology service to their preferred place of care.

Key Responsibilities
  • Provide senior support to the multidisciplinary team to facilitate safe and timely discharges for patients back to the community, referring to hospitals, hospices, residential care or other preferred places of care.
  • Collaborate with patients, carers, and healthcare professionals to identify and address any barriers to discharge.
  • Develop and maintain effective relationships with community healthcare providers to ensure seamless transitions of care.
  • Conduct thorough assessments of patients' needs and develop personalized discharge plans.
  • Monitor and evaluate the effectiveness of discharge plans and make adjustments as necessary.
Requirements
  • Registered Nurse Part 1, 2 or relevant section of the register.
  • Experience of working within a busy day-to-day NHS service.
  • Prior relevant experience of working with cancer services.
  • Experience of working directly with patients/clients with complex health needs and/or housing/social placement issues.
  • Demonstrates awareness of the importance of working as part of a multi-disciplinary team.
  • Communicates effectively verbally, in writing and in electronic formats.
