West London Trust are embarking on an exciting new pilot service for the winter months to strengthen and improve our oversight of flow within the acute and crisis care pathway.

We are looking for exceptionally motivated staff who enjoy working at pace with excellent communication skills. This service will initially run from October 23 to March 24 and we are keen to attract staff externally and internally, therefore are offering secondments and some permanent / fixed term roles. Over the course of this pilot service we will discuss your development and career aspirations and you will have access to the Trusts learning and development opportunities through the Personal Development Review (PDR) process. Towards the review in March 24 we will work with you to understand how we can support your career within West London Trust and be able to determine if the service will continue or identify a number of roles available to consider as your follow on from this pilot service. Staff can be employed permanently and will be fully supported to find the right follow on role for them. West London are keen to work with staff in order for them to grow and develop within our organisation through offering career development and a Talent Pool programme.

The Post holder would ideally have experience working as a care or discharge coordinator within West London or other Trusts or similar environments.

The post holder will work closely with both our Acute Mental Health Units in identifying any barriers to discharge upon admission implementing a least restrictive process model. They will work 'hand in hand' with their colleagues, other community teams in and out of the trust, primary care colleagues and the patient's family/carers to facilitate discharge. They will ensure that the estimated discharge date as identified during reviews is progressed where possible and feasible.

You would be expected to provide accurate information for the daily discharge meeting directly, or to ensure this information is relayed through ward staff and junior members of the team.

This is a unique role that offers exposure to skills development across services, business functions and discharge coordination through means of providing adequate social care support

**Key Result Areas & Performance**:
Admission Stage

Work with the team manager to establish barriers to discharge and create links with the MDT, patient and care coordinator.

Daily meeting to discuss all new admissions and start discharge planning

Treatment / Review Stages

Attend ward reviews and S117 meetings where critical decisions are made in relation to cases assigned in order to facilitate timely discharge.

Promote, participate and support the patients in MDT reviews

Consider and support practical tasks to overcome barriers to discharge in collaboration with the inpatient MDT and community team.

Approach to Discharge

Resolve arising barriers to discharge i.e. keys, other agency support to discharge.

Ensure early liaison with the Recovery teams, CATT and the local authority to allow for adequate post-discharge support.
