Discharge Planning Coordinator

iPeople Solutions is recruiting an experienced Hospital Social Worker to work in North London, focusing on discharge planning and coordination for adults in need. The successful candidate will have a strong understanding of adult social care principles and practices, working closely with local authorities and healthcare professionals.

This role involves conducting assessments, developing support plans, and collaborating with patients, families, and carers to achieve seamless transitions from hospital to community settings. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work flexibly and manage competing priorities.

Responsibilities:

  • Conduct thorough assessments to identify individual needs and develop personalized support plans
  • Collaborate with multidisciplinary teams to ensure smooth patient discharges and effective community reintegration
  • Provide expert guidance on accessing community resources and services to promote independence
  • Maintain accurate records and contribute to policy development and service enhancement

Essential Qualifications:

  • Degree in Social Work or related field
  • HCPC registration (or willingness to register)
  • Strong communication and interpersonal skills
  • Ability to work flexibly and manage competing priorities

Compensation Package: £45,000 - £55,000 per annum
