Hospital Discharge Social Worker

Entrust Social Care Ltd is looking for an experienced Hospital Discharge Social Worker to join their team in Bracknell Forest Council. In this role, you will work as part of a multi-disciplinary team to support older people with diagnosed memory problems, ensuring their social care needs are met within eligibility criteria.
You will oversee hospital discharges under the Discharge to Assess Framework / Care Act, working collaboratively with healthcare and social care colleagues to respond to urgent requests for assessment/monitoring/support. The successful candidate will also work with the intermediate care team to facilitate reablement when identified and partner with service users, carers, professionals, and agencies to meet social care needs.
This is a full-time position with a competitive salary, weekly payroll, free access to an online training portal worth £500 a year, and a dedicated consultant with social work knowledge. To be successful in this role, you must have DipSW or equivalent, HCPC registration, current DBS, and a car driver license.
