About the Role:

  • You will work closely with healthcare professionals and social care teams to coordinate hospital discharges.
  • You will develop and implement effective discharge plans that meet the needs of residents.
  • You will maintain accurate records and reports to track progress and identify areas for improvement.

Key Responsibilities:

  • Coordinate hospital discharges to ensure seamless transitions to community care.
  • Develop and implement individualized discharge plans that prioritize resident needs.
  • Maintain accurate records and reports to inform quality improvement initiatives.

Requirements:

  • You should have a strong understanding of health and social care systems and processes.
