Intermediate Care Discharge Planner

We are seeking an experienced Social Worker to join our team as an Intermediate Care Discharge Planner. In this role, you will be responsible for supporting patients' transitions from hospital to community settings, ensuring seamless care coordination and developing effective care plans.

Key Responsibilities:
  • Work closely with healthcare professionals and families to identify patients' needs and develop personalized care plans.
  • Collaborate with community services to ensure timely discharges and effective care coordination.
Requirements:
  • Hold a valid Social Work Qualification and be registered with Social Work England.
  • Possess a valid DBS Check, either within 12 months or registered to the Update Service.

We offer a competitive salary of £52,000 - £62,000 per annum, depending on experience, as well as opportunities for professional growth and development in a dynamic and supportive environment.
