Detailed job specifications include:

  • Job Title - Registered Social Worker (MUST have 2 years post AYSE experience)
  • Location - Havering
  • Duration - 3 months ongoing
  • Schedule - Monday to Friday, 9am to 5pm

The estimated monthly salary for this position is approximately £13,784 - £15,096 (based on 12 months and 30 hours of work per week), offering a competitive rate for experienced professionals.

Primary Job Functions

This role involves evaluating patients' needs and those of their caregivers, implementing effective care strategies, fostering strong relationships with healthcare professionals and external agencies, ensuring seamless transitions from hospital care, and taking proactive measures to prevent readmissions.
