Discharge Coordinator

An opportunity has arisen to join our established team of Specialist Discharge Nurses and Coordinators working across our 3 sites in Imperial Healthcare NHS Trust. This role provides the opportunity to make a difference to patient experience at the end of their journey in hospital.
This is a challenging role requiring the post holder to work closely with the multi-disciplinary team patients and their families, to support the discharge of complex patients from acute care. It is a varied and challenging role which encourages you to use a problem-solving approach and to think outside the box.
A broad overview of responsibilities includes providing support to and working extensively with multi-disciplinary and multi-agency staff to co-ordinate complex discharge planning for those patients who require on-going care following discharge from hospital.
You will become part of a team with a wealth of experience, both clinically and administratively, who will be able to offer you support, training and guidance as you develop your career with us. Experience working in the acute health care or adult social care sector would be an advantage.
You will be based at St Mary’s hospital but will provide cross cover to the other sites if required.
The post holder works alongside the Specialist Discharge Coordinators and Specialist Discharge Nurses to support the Trusts agenda to minimise delayed discharges. The role will involve responsibility for coordinating complex patient discharges. This will be achieved through attending ward multidisciplinary meetings and daily ward visits and close working with patients and their carers, nursing staff and the multidisciplinary team.
