We are seeking a highly skilled and motivated individual to join our team as a Discharge Liaison Coordinator at our Holybourne Hospital in Roehampton. As a Hospital Administrator with Active Care Group Recruitment, you will have the opportunity to work in a dynamic and supportive environment.

Job Description

The successful candidate will be responsible for:

  1. Attending community meetings and providing minute taking services.
  2. Maintaining electronic action logs for community meetings.
  3. Providing administration support to the wards and taking part in the rota covering reception duty.
  4. Preparing summaries for the wards to present at Clinical Governance meetings of the monthly data (incidents analysis, risks, good practice etc).
  5. Maintaining a database of CPA and Discharge activity within the service.

You will work closely with the ward teams to ensure that clinical documents are available pre and post CPA's and Discharge meetings. The ideal candidate will have excellent communication skills and be able to maintain confidentiality.

We offer a competitive salary of £33,500 - £36,000 per annum depending on experience, and a comprehensive benefits package including 25 days plus bank holidays, birthday off, and Enhanced Sick & Maternity Pay benefits.
