Discharge Planning Specialist Job Summary

We are seeking an experienced Discharge Planning Specialist to join our Case Management Team at HCA Healthcare UK. As a key member of the team, you will play a vital role in assessing, planning, implementing, and evaluating care options and services that meet patients' health needs.

About the Role:

The successful candidate will have experience in Inpatient Bedside Nursing for Adults & Paediatrics, with a proven track record in utilisation review and discharge planning. They will be an excellent communicator, with strong organisational and computer skills, and a dynamic approach to problem solving.

Responsibilities:

  • Develop and implement individualised discharge plans, in collaboration with multidisciplinary teams
  • Coordinate proactive comprehensive discharge planning activities, ensuring seamless transitions between care settings
  • Act as a liaison between patients, hospitals, physicians, and third-party payers, optimising community treatment/services as required

Requirements:

  • NMC Registered Nurse with substantial post-graduate experience (Adult registered with Paediatric Experience)
  • Experience of utilisation review and extensive discharge planning experience essential
  • Experience as a Senior Staff Nurse/ band 6 or above essential

Salary: £60,000 - £75,000 per annum + benefits
