About the Role

An exciting opportunity has arisen for a skilled professional to join our team as a Discharge Planning Specialist. This role is based at The Royal London Hospital site and involves supporting ward staff in facilitating and planning patient discharge.

As a key member of our multidisciplinary team, you will engage with patients and their next of kin to empower them to actively participate and prepare for discharge from the ward.

We are focused on delivering high-quality patient care, and this role plays a crucial part in achieving that goal. You will work closely with clinicians, therapists, specialist older persons teams, social workers, and the complex discharge team to ensure seamless patient transitions.

The post holder will champion the use of our internal patient record system, recording live updates in relation to discharge throughout the day and following key patient meetings.

Main Responsibilities
  • To provide expert support to ward staff in planning and facilitating patient discharge
  • To engage with patients and their next of kin to empower them to actively participate in discharge preparation
  • To work collaboratively with the multidisciplinary team to ensure effective patient transitions

This role requires a unique blend of skills and experience, including GCSE Grade A-C in Maths & English or equivalent qualifications, and a willingness to undertake further study in areas relevant to the role applied for.

We offer a competitive salary of £25,000 - £30,000 per annum, depending on experience, as well as a range of benefits including access to training and development opportunities, and a supportive working environment.
